Setting up Patient Records
For MOH claiming to work correctly, each eligible patient needs:
NHI Number: The patient's unique National Health Index number, entered in their patient profile.
School: The patient's school must be recorded in their profile settings.
This determines the pricing band (COM1, COM2, or COM3) based on the school's Equity Index (EQI), which drives the fee schedule for the OHSA Standard Package.
These settings can be found on the patient's profile.
đĄTip If school information is not yet available for a patient, you can temporarily set it to "not enrolled".
MOH Enrolment Forms
Sending an enrolment form:
Open the Patient Profile. Located at the bottom left of the profile will be a section titled 'MOH'.
2. Click 'Send MOH Enrolment Form' to open the form.
Most fields will auto-populate with the patient's details from their profile, including:
Name and contact details
NHI number
School information
The only field you may need to manually enter is the school year.
3. Click 'Generate Form' to create the completed form.
4. Add signatures before clicking 'Send Forms'.
đĄThe form will be sent to the enrolment email address configured in your Practice Integrations (Settings > Practice > Integrations).
