You can send the Medical History Form via SMS or email to your patient prior to the patient's arrival.
You can do this either by automated SMS or email or by sending a one off SMS or email.

Sending forms via automated SMS & emails
Automated SMS and emails are created in the treatment template. Click here to learn how to set this up.
Sending forms via one off SMS & emails
First you will need to create the SMS or email template to include the unique patient medical form link. Click here to learn how to create document templates.
Be sure to select APPOINTMENT as the scope of the template.
Then, to send off the one-off link, find the patient's appointment in either the Timeline or Receptionist Dashboard view.
Click the 3 dot menu icon.
Select SMS or EMAIL, then select the template in the drop-down menu.

Forms Reminder
You can also remind patients to complete forms such as a Consent Form or Medical History Form as part of the Check-In checklist completed by the receptionist at Check-In. These checklists are treatment specific and should be configured to the type of treatment at treatment creation.
